Holiday Club Registration 2019

Are you ready to explore the Australian Outback? We are looking for "explorers" in Kindy through to year 6 in 2019 to ‘Get lost in the outback’!

Our annual kids holiday club is back again this January with more fun than ever! Register your kids for 5 mornings packed full of crafts, cooking, games, dramas, talks, small group time and delicious morning tea all set within our exciting theme of the Australian Outback.  All primary aged children are welcome to join us!

Your children don’t get all the fun though. We also have a delicious parent café which runs every afternoon from 12noon -pickup time. Come and enjoy a coffee alongside some tasty sweets and see what the children have been up to. We would love to see you there!

For children in Kindy to Year 6 in 2019          Monday 14th – Friday 18th January 2019         9am-12:30pm

Location: St Aidan’s Anglican Church, Hurstville Grove – Cnr Hillcrest Ave and Lansdowne St           

 Cost: $15 per child per day ($75 for the week). 

Early bird rate: Receive 1 day free ($60 for the week) when you register your child for all 5 days before midnight 14th Dec 2018.

Family discount: Receive a 10 % discount when you register 3 or more children in one family (applies on top of early bird weekly rate).

Authorisations and Expectations

I give permission for photos and videos of my child to be taken at Holiday Club to be displayed publicly (online and in print) unless I advise Holiday Club Directors.         Click here  to email them now, or later at staidansholidayclub@gmail.com.

 

  • I give permission for my child to attend all scheduled 2019 St Aidan’s Holliday Club activities, unless I advise otherwise.  This includes ‘Park Day’ to McRaes Park on Thursday January 17th, which involves walking between the park and the church.
  • I understand all Holiday Club leaders and helpers have completed a ‘Safe Ministry’ course provided by Anglican Diocese.
  • I understand that leaders and helpers over the age of 18 years have a cleared ‘Working With Children’ number, allowing them to care for and interact with children.
  • I understand that my child will be under the supervision of Emily Cartwright and Greg Ball (Holiday Club Directors) throughout the Holiday Club program
  • I authorise Holiday Club First Aiders, in the event of an emergency, to obtain at my expense any medical, ambulance, rescue or other services that are considered necessary for my child.
  • I understand my child may need to be driven in vehicles in the case of medical attention being needed, I give permission for my child to be driven by individuals who hold a Provisional P2 Licence (“Green Ps”) or Full Licence. 
  • I acknowledge that being part of a community involves mutual care and consideration , and therefore agree that unacceptable behavior may result in my child being sent home and being temporarily or permanently prohibited from attending the activities of Holiday Club 2019.
  • I will provide the Holiday Club Directors with any information relevant to the wellbeing of my child prior to him or her attending a Holiday Club 2019 activity.
  • I confirm that this information given in this form is true and correct, and will advise Holiday Club Directors of any changes to this information.

The information collected on this form will be use for the purpose of supporting children at Holiday Club on behalf of St Aidan's Anglican Church Hurstville Grove. It will be disclosed to the Holiday Club Directors and first aider during the program.  It may be shared with the Holiday Club caterers, small group leader for the child/ren, if deemed necessary to better support the child/ren in the program, external providers of programs or health professionals as required to support the child/ren. It will be handled in accordance with our privacy policy. By submitting this information by paper or electronic means you indicate you consent for us to collect, use and disclose the information you have provided in this manner.
